HEALTHAlliance Benefit Plan offers two dental plans through Delta Dental. Both of our plans provide:

• 100% of routine preventive and diagnostic care such as cleanings and exams.

• 90% of basic procedures such as fillings, root canals and tooth extractions when visiting an in-network provider; 80% is covered when visiting a non-network provider.

• 50% of major services such as crowns, bridges and implants.

​

• Annual Maximum Carryover – To GoSM for carryover of unused Benefit Period Maximums to the next benefit contract year.
